What is Integrated Care?
Integrated care happens when a team of Primary Care Providers (PCPs) and behavioral health providers work with Members and their families. The goal is a patient and family-centered approach to address whole person health. Providers who coordinate Members’ mental and physical health treatment help the Member have the best health outcomes.
A persons’ behavioral health connects to their physical health (and vice-versa). Integrated care has a positive impact on whole person health such as:
- Decreased patient depression levels;
- Improved quality of life;
- Decreased stress;
- Lower rates of hospitalization
Integrated health treatment may improve Member and provider satisfaction by reducing access barriers and improving communication between multiple providers.
